The HIPAA privacy rule was brought into force as a means of regulating how “Covered Entities” store, disclose and use information that is classed as PHI or Protected Health Information. HIPAA privacy rule’s effective date of compliance was on the 14th of April, 2003, and a particular class of ‘small plans’ getting a one-year extension to comply.
This “Covered Entities” classification encompasses organizations such as health insurers, medical service providers, health care clearing houses, health insurers, health care plans which are sponsored by the employer, and later on extended to another group referenced under HIPAA privacy rule summary as the “business associates” of ‘Covered Entities’. Atrial Fibrillation is also known as A-fib or AF. An AF is essentially an abnormal rhythm in the heart’s beating pattern—atrial fibrillation represents a serious cardiovascular problem and is often a fatal condition. However, this doesn’t mean that all cases of Atrial Fibrillation lead to fatalities. It has been noticed that most people suffering from Atrial Fibrillation are in the age-group of 60 years and over. There are some other patterns in Heart Attack Atrial Fibrillation too where it is witnessed more frequently among middle-aged and grown men as compared to women.
Understanding Atrial Fibrillation
To understand AF, the functioning of the heart needs to be understood. Firstly, the heart pumps oxygen-enriched blood across the body using network of blood vessels like the arteries. This ensures that all the organs of the body apart from muscles and nerves are supplied with sufficient oxygen to sustain their active, functional status. The pattern in which the heart beats is further reflected in the form of small electric pulses that are indicative of the overall health of the cardiac chamber. The standard electrical pattern of heartbeat begins when electrical impulses are produced by the sinus node. This electric impulse is further conducted across the top chambers of the heart. This is how the heart contracts, ensuring sufficient pressure with which the blood is pumped across the body.
When an Atrial Fibrillation develops, there is a disturbance in the rhythm of the heart. There are many kinds of AFs and each of these represents a different degree of threat in term of fatality prospects and or the degree of discomfort it induces. In most cases of Atrial Fibrillation, the upper chambers of the heart, i.e. the atrium, don’t function properly. Please note that unlike heart failures or heart attacks symptoms, AF takes a much longer time to develop. This is because the heart continuously strives to maintain its regular electrical conductivity. When an abnormal rhythm develops in the heart, oxygenated blood is moved out of the heart’s chambers with insufficient force.
This means that the atria have become sluggish. This allows some of the blood to be retained or flow back into the heart. This raises the risk of blood-clotting and can even cause the heart to stop beating for a few seconds. There are many ways in which an individual can suffer Atrial Fibrillation. Many times AF attacks continue for days, while sometimes they are acute and sometimes they are over in a few hours.
Common Types of Atrial Fibrillation:
- Intermittent Atrial Fibrillation-this is also called paroxysmal AF and is characterized by AF episodes of varying frequency, each lasting for an inconsistent period before ceasing simultaneously.
- Chronic Atrial Fibrillation-this is the more long-lasting or persistent types of AFT that is sustained over a long period and doesn’t stop immediately.
Atrial Fibrillation is more likely to follow a heart attack. Again, its’ incidence among people who have been diagnosed with cardiovascular diseases or those who have an undetected heart disease are more likely to fall victims to Atrial Fibrillation. Other, common causes of Atrial Fibrillation include:
- Myocardial Infarction (MI)
- Congestive Heart failure (CHF)
- Chronically High Blood Pressure
- Heart Valve Disease (like mitral stenosis)

Low Progesterone Symptoms

HIPAA 5010 Extended Until March 2012

All covered entities, including hospitals, physician offices and their business associates, now have an extended, 45 days to ensure their compliance with HIPAA 5010. CMS had set January 1, 2012 as the final date for being HIPAA 5010 compliant but now, this has been revised to the end of March 2012.
HIPAA 5010 extension is mainly a result of widespread industry feedback which suggested that healthcare organizations, mainly private offices, had been unable to find suitable software solutions. Vendors too had been a bit laidback in updating their software solutions. OESS wanted to ensure that all physician offices were aligned together, using the new 5010 format for submitting their claims rather than using both 5010 and 4010 formats or using 4010 format only which could lead to outright rejection claims by clearinghouses and extra re-processing costs.
Covered entities that have been unable to meet the standards set for electronic transaction processing as per HIPAA 5010 categorized under NCPDP Medicaid Subrogation 3.0 (NCPDP 3.0), ASC X12 Version 5010 and NCPDP Telecom D.0 shouldn’t feel relaxed with this extension. The OESS (Office of eHealth Standards and Services) will continue accepting complaints that suggest non-compliance with HIPAA 5010 by any of them. OESS can even ask the covered entities to substantiate their efforts towards achieving HIPAA 5010 compliance during this extension period.
